PCOS treatment in India
The treatment of Polycystic Ovarian Syndrome (PCOS) is often customized based on your symptoms, health objectives (fertility or otherwise), and risk factors. As it is a hormonal and metabolic disease, it may be treated with lifestyle modifications, medications and occasionally fertility treatments.
Lifestyle/Natural Management (First-line Treatment)
-
Weight control: A 5-10 percent reduction is enough to positively affect the ovulation, insulin resistance, and hormone balance.
Diet:
- Whole grains and legumes (low-glycemic index (GI) foods).
- Slim meat (fish, eggs, chicken).
- Vegetables and fruits containing fiber.
- Cut back sugar, processed foods, refined carbohydrates.
- Exercise: 30 to 40 minutes of moderate exercise (walking, cycling, yoga, strength training) 4-5 days/week.
- Stress management: Yoga, meditation, and good sleep hygiene will help in hormonal regulation.
Medical Therapy (Symptom-based)
Menstrual problems and hormonal disorder
- Combined oral contraceptives pills (COCPs) - control menstruation, lower the occurrence of acne and excess hair growth.
- Progestin therapy - helps to guard against the uterine lining when you are not having regular periods.
Insulin resistance & metabolic problems
- Metformin - increases insulin sensitivity, assists in regulating cycles and could contribute to weight loss.
- Hair and acne excess (Hyperandrogenism)
- Anti-androgens (spironolactone, flutamide, finasteride - only with a prescription).
- Hair removal by topical creams, laser, or electrolysis.
Fertility-focused Treatment
If pregnancy is the goal:
Ovulation induction medicine:
- Letrozole (first choice) or Clomiphene citrate.
- Gonadotropins not in tablet form.
- Metformin - can be used together with ovulation medicines.
- Assisted Reproductive Techniques (ART)
- IVF (In Vitro Fertilization) when medicines do not work.
Surgery (Rarely used now)
-
Laparoscopic ovarian drilling (LOD) - minute piercings are made in ovaries to reduce the levels of androgens and induce ovulation. Only used when drugs do not work.
Long-term Health Monitoring
Women who are PCOS are at increased risk of:
- Type 2 diabetes
- High cholesterol
- High blood pressure
- Endometrial cancer (when the periods are irregular over years)
- It is important to monitor the level of blood sugar, cholesterol, and endometrial health regularly.

Factors Affecting PCOS treatment cost in India
The main determinants of the price of PCOS treatment in India, including the specific numbers where they are known:
Principal Cost Drivers
Diagnostic Testing
Basic tests:
- Pelvic ultrasound
- Blood tests (Hormonal LH, FSH, testosterone)
Advanced tests:
- Transvaginal ultrasound
- Extensive hormone analyses such as AMH, insulin, thyroid, prolactin.
- The overall diagnostic cost is more variable based on scope and use by a private or government clinic.
Consultation & Follow-Ups
Doctor’s fee varies:
- Initial visit
- Follow-ups
- Pricing is conditioned by clinic reputation and location (urban and smaller towns).
Type & Duration of Treatment
The administration of medications (monthly):
- Combined oral contraceptives / hormonal therapy.
- Lifestyle & nutrition interventions (3 6 months)
- Wellness packages of the skin or weight.
Fertility Treatments
-
Ovulation induction & IVF (if needed) may increase the total cost.
Surgical Interventions (Uncommon)
-
Laparoscopic ovarian drilling may cost more.
Location & Type of healthcare facility
- In metro cities such as Mumbai, Delhi, Bangalore: cost of treatment is higher on average.
- Smaller urban areas (e.g. Pune, Chennai) are cheaper.
Severity & Complexity of Condition
- Mild cases: fewer tests and lifestyle change only will make it cheaper.
- Severe or infertility-oriented cases: repeated tests, drugs, fertility drugs or surgeries = increased cumulative expenditure.
Long-Term Management and follow-up
-
PCOS may involve continuous monitoring and treatment-expenses in the long term add up, particularly hormonal medications, regular check-ups and doctor appointments.
PCOS fertility treatment in India
Lifestyle First
- The first-line therapy is weight reduction and lifestyle changes, particularly in women with a BMI 25 and above (in Asian Indians).
- A weight reduction as small as 5% can go a long way in reinstating ovulation and increasing the sensitivity of fertility therapy.
Ovulation Induction drugs
- Aromatase (Letrozole) has become the new drug of choice to promote ovulation in women with PCOS--it has superior pregnancy and live birth outcomes than clomiphene citrate (CC) and is now preferable as the first-line drug.
- Clomiphene citrate (CC) can be considered as an option to letrozole as long as it is not viable.
- The frequently used insulin-sensitizing drug, Metformin, could potentially improve pregnancy outcomes with CC, but not alone, ovulation induction will not work well in the case of Metformin.
Advanced Approaches
- Injectable FSH/LH (gonadotropins) are applied in the event of failure of oral medications, but they must be closely monitored through ultrasound and are associated with increased multiple pregnancies.
- Laparoscopic ovarian drilling is not only a surgical alternative that can restore ovulation in CC-resistant anovulation, but also carries a reduced risk of multiple pregnancies than gonadotropins.
Assisted Reproductive Technologies (ART)
- Ovulation induction medications are usually combined with Intrauterine Insemination (IUI) when there is subfertility.
- IVF is a third-line treatment, should all the other methods be unsuccessful, and plans such as freezing-all cycles to reduce the threat of OHSS are in place.
- IVM could replace complete IVF in PCOS. It is not high-dose stimulating, has a lower risk of OHSS, and may even prove to be more affordable.
