Intrauterine insemination
Intrauterine Insemination (IUI) refers to a fertility procedure whereby specially prepared sperm are directly introduced into the uterus of a woman during or around ovulation in order to enhance the likelihood of pregnancy. It is the less invasive and cheaper alternative to in-vitro fertilization (IVF).
What is IUI?
- A fertility treatment in which treated and concentrated sperm are introduced into the uterus by a slender catheter.
- The idea is to bring the sperm nearer to the egg in the fallopian tube and shorten the distance they have to cover.
When is IUI Recommended?
- Unexplained infertility
- Male infertility with mild male factor (reduced number or motility of sperm)
- Cervical factor infertility (thick cervical mucus)
- Ovulation issues (e.g. irregular ovulation)
- Ejaculation issues
- Use of donor sperm
Steps in the IUI Procedure:
Ovulation Monitoring
- Fertility drug (clomiphene, letrozole, or gonadotropins) therapy on natural cycle or ovulation induction.
- Ultrasound and blood tests monitor the progress of the egg and ovulation time.
Sperm Preparation
- Sperm sample taken by the partner/donor.
- Sperm washing: this process cleans the sperm and purifies it.
Insemination
- Carried out in a clinic, not very painful.
- Catheter inserted in an attempt to place sperm in the uterus.
After Procedure
- Short-term rest of patient.
- Usually normal activities can be reinstated the same day.
Success Rates
- In general 1020 percent per cycle, with age, fertility problems, and drug use.
- Repeat cycles may be required (usually up to 36).
Advantages
- Not as intrusive or expensive as IVF.
- Easy outpatient surgery.
- Can be used along with fertility drugs to have more success.
Intrauterine insemination treatment in India
Initial Evaluation
-
Fertility specialist captures medical history of the two partners.
Tests may include:
- Semen analysis (to test sperm count and motility).
- Hormonal tests.
- Ultrasound to test ovaries and uterus.
- Tubal patency test (to check that fallopian tubes are open).
Ovarian Stimulation (where necessary)
- Women can use oral or injectable fertility medication (Clomiphene, Letrozole or Gonadotropins).
- Purpose: to provoke the ovaries to work out 1-2 mature ovules.
- Frequent ultrasound and blood testing to measure follicle development.
Ovulation Trigger
- HCG trigger injection can be provided when the follicles have the appropriate size (typically 18-20 mm).
- This allows the egg to be discharged when it is of the appropriate time to get pregnant.
Sperm Collection/ Preparation
-
Partner/donor gives a sample of the semen.
Sample gets sperm washed:
- Expels seminal fluid and sperm that is immobile.
- Precious, active sperm.
Insemination Procedure
- A slender and pliable catheter is inserted where the prepared sperm is introduced directly into the uterus.
- Procedure is rapid (5-10 minutes) and is not painful (as with a Pap smear).
- No anaesthesia is needed.
Post-Procedure
- Woman may rest for 15–30 minutes.
- Normalization may then resume.
- A little mild cramping or spotting may take place.
Luteal Phase Support
-
In rare cases progesterone supplements may be given to aid implantation.
Pregnancy Test
-
Approximately 2 weeks later a blood test is performed to establish pregnancy.

Factors Affecting Cost of intrauterine insemination treatment in India
Important variables that affect the prices of intrauterine insemination (IUI) treatment in India, as well as average prices by city:
Location & Clinic Type
- Metro city clinics (Delhi, Mumbai, Bangalore, Hyderabad, Chennai) are more expensive because they have more overheads and better infrastructure.
- Well-known or premium clinics and hospitals are usually more expensive compared to smaller or rural clinics and hospitals.
Specialization of the doctor and clinic
-
One can get treatment with highly skilled fertility specialists at a high price but with possibly better results.
Number of Cycles Required
-
It usually takes many cycles of IUI (3-6) to conceive. The costs of the full procedure are incurred in each cycle.
Drugs (Ovulation Stimulation & Trigger Shots)
-
The cost also differs depending on the type of oral agent (e.g. Clomid), injectables, or imported drugs which greatly increase costs.
Monitoring & Diagnostic Tests
-
Involves a blood test, semen test, a series of ultrasounds to track the follicle, and possibly other more advanced tests (e.g., sperm DNA fragmentation). These put a dent on the expense.
Donor Sperm & Add-On Services
-
The overall cost is augmented by using donor sperm, or spermic freezing, or sophisticated methods of monitoring.
Women’s Age & Medical History
-
Aging or complicated causes of infertility also lower the success rate per cycle with many requiring multiple and further procedures.
Miscellaneous Costs and Operating Costs
-
Certain extensive packages can also cover hospital care, nursing, travel assistance or could be part of medical tourism packages which can influence overall billing.
