Diagnostic Laparoscopy in India

A diagnostic laparoscopy is a surgery that is minimally invasive and enables the doctor to see the organs directly in and around the abdomen and the pelvis. The laparoscope (a thin lighted telescope like instrument) is used to perform it through a small opening typically located near the navel. The laparoscope sends the pictures to a screen, which allows the surgeon to examine the internal organs without necessarily causing huge cuts.

Purpose

Diagnostic laparoscopy is typically applied to:

  • Explore the mystery of abdominal or pelvic pains.
  • Identify infertility among women.

Diagnose and assess such disorders as:

  • Endometriosis
  • Pelvic inflammatory disease (PID).
  • Adhesions (scar tissue)
  • Ovarian cysts or tumors
  • Ectopic pregnancy
  • Determine metastasis (staging) of abdominal or pelvic cancers.
  • Assess injuries or internal haemorrhage.

Procedure

  • The patient is subjected to general anaesthesia.
  • A cut (0.51 cm) is made around the belly button.
  • The abdomen is inflated by introducing carbon dioxide gas in order to enhance visibility and space to work.
  • Laparoscope is inserted and the surgeon observes the organs of the abdominal and pelvis on video screen.
  • Further minor cuts can be taken in case of the necessity of surgical tools.
  • Examination leads to the release of the gas and sutures or surgical tapes are used to close the incisions.

Advantages

  • Minimally invasive (smaller incisions than those in open surgery).
  • Reduced hospitalization and quick recovery.
  • Less pain and scarring.
  • Exact and straight forward visualization of internal organs.

Factors Affecting Cost of Diagnostic Laparoscopy in India

The following are the key issues that influence the cost of a diagnostic laparoscopy in India:

Type/Infrastructure of the hospital

  • Hospitals with premium and multispecialty will be more expensive. Smaller hospitals/clinics with less complex facilities tend to be less expensive.

City / Location

  • Metros (Delhi, Mumbai, Bangalore, etc.) are more expensive because the overheads (rent, salaries of staff members, utilities) are higher. Less in smaller cities/towns.

Experience and reputation of the surgeon

  • Surgeons who are more experienced / well-know are pricier in terms of professional fees.

Pre-operative diagnostic and tests

  • Blood tests, Imaging (ultrasound, CT/MRI), ECG etc. These are cost additions depending on the requirements. In case certain tests are complicated or more than one, the price increases.

Anaesthesia type

  • General anaesthesia is more costly than local/regional and the time is important. The more complicated or a lengthy process, the higher the cost of anaesthesia.

Time and Complexity of the Process

  • In case the diagnostics laparoscopy reveals a problem that requires further treatment (e.g. ballooning, adhesions, cysts) or even has to be changed to therapeutic laparoscopy, the costs rise.

Hospital stay / Room type

  • Higher costs in case patient has to spend an overnight or more, or is moving to a higher quality of room (private/deluxe). Also price of after-operative treatment.

Consumables, equipment and disposables

  • Laparoscope, surgical equipment, single-use products, sutures, etc. High quality/disposable products cost higher. Also the maintenance expenses of instruments.

Medicines / postoperative treatment

  • Painkillers, antibiotics, follow-ups, dressings etc. Where complications arise or care is extended, the cost will rise.

Diagnostic Laparoscopy Procedure in India

This is a neat step-wise process of the Diagnostic Laparoscopy Procedure:

Preparation

  • Fasting: The patient should avoid any consumption of food or beverage 6-8 hours prior to surgery.
  • Pre-op tests: Pre-op tests typically include blood work, ECG, imaging and urine tests.
  • Consent: The physician describes the risks/benefits and acquires a written consent.
  • Anaesthesia: The procedure is nearly always performed with the aid of general anaesthesia.

Procedure Steps

  • Positioning: The patient is placed on the operating table, most of the time in the supine (lying on his back) position.
  • Incision: Near the navel, a small cut (approximately 0.51 cm) is made.
  • Gas Insufflation: This involves pumping of gas into the abdomen to inflate it using carbon dioxide. This provides clearance to the surgeon to view organs.
  • Insertion of Laparoscope: Laparoscope is a thin tube with camera and light that is inserted through the opening. The pictures are displayed on a screen.
  • Examination: The surgeon closely looks at abdominal and pelvic organs including:
  • Liver, stomach, intestines
  • Gallbladder, appendix
  • Fallopian tubes, ovaries (women), uterus.
  • Additional Ports (when necessary): In case of a necessity more instruments are required, 1 2 small incisions could be made.
  • Biopsy (when necessary): A small piece of tissue can be obtained and subjected to laboratory testing.
  • Completion: The instruments are removed after checking.
  • Closure: The gas is exposed, and the minor incisions are sewn using the suture or umbilical tape.

Duration

  • Typically 30-60 minutes based on complexity.

Recovery

  • The patient is placed under observation in the recovery room until he is awake due to anaesthesia.
  • The discharge is frequently on the same or the following day.
  • The mild pain, bloating or shoulder pain (as a result of the gas) may take 1–2 days.
  • Complete recovery in 3-7 days.
