What is Bone Marrow Transplant?
BMT is one of the treatments through which damaged or diseased bone marrow is replaced by stem cells from the patient’s own body or a donor. It is also referred to as a stem cell transplant.
Bone Marrow Transplant Procedures
BMT involves a series of medical procedures in which the patient’s damaged or diseased bone marrow is replaced with healthy stem cells. The following is an elaborative analysis of the process stated above:
Pre-Transplant Evaluation & Preparation
Blood Tests, Bone Marrow Biopsy, Imaging Tests, Heart and Lung Function Tests, HLA typing Stem Cell Collection Depending on the type of transplant, stem cell harvest stemmed from the following:
- The patient (Autologous BMT) – This is attained through collection of stem cells before the chemotherapy treatment is administered.
- A matched donor is also known as Allogeneic BMT- it can be harvested from the bone marrow, peripheral blood or umbilical cord Blood.
Stem cell collection methods:
- Bone Marrow Harvesting – The stem cells are collected from the hip bone of the donor when he or she is undergoing surgery or under general anaesthesia.
- Peripheral Blood Stem Cell Collection – The donor is given growth factors with the intention of stimulating the number of stem cells in the bloodstream and these cells are harvested through a process called apheresis.
Transplant (Stem Cell Infusion)
- The collected stem cells are then transplanted through IV line (central venous catheter) which is done in a manner similar to that of blood transfusion.
- This process will take a few hours and it is not very discomforting.
- It reaches the bone marrow area and proceeds to produce healthy blood cells.
Post-Transplant Recovery & Engraftment
Engraftment: The transplanted stem cells start differentiating into blood cells, which is usually between ten to thirty days.
The following are potential complications checked closely among the patients:
- Infections (due to low immunity)
- Graft-versus-host disease (GVHD) (in allogeneic transplants)
- Organ damage from chemotherapy or radiation
Supportive Care:
- A possible procedure might require a transfusion of blood and / or platelets.
- Those include antibiotics, antifungals, immunosuppressant drugs among other.
- In order to reduce their exposure to different patients, a patient can spend several weeks in a solitary patient room in a hospital.
Long-Term Recovery & Follow-up
- Complete recovery of the immune system might take 6 months to 1 year for chicken pox disease.
- One has to have routine blood checks and other general check-ups.
- A patient should not attend congested areas, take hygienic food and adhere to the word of the doctor in order to avoid contracting an infection.
Bone Marrow Transplant Treatment
Bone marrow transplant or BMT is a process through which the patient’s unhealthy or damaged bone marrow is replaced with healthy stem cells. It is used in several illnesses once relating to blood cancer such as leukemia, Lymphoma, multiple myeloma, blood diseases including thalassemia, sickle cell anemia, aplastic anemia, and immune system diseases.
Types of Bone Marrow Transplant
The results of the transplant depend on the path that has been chosen for the patient:
Autologous BMT (Self-Transplant)
- The patient’s own stem cells need to be harvested before the chemotherapy or radiation is administered.
- Most mainly used in blood cancers namely lymphoma and multiple myeloma.
- Lower rate of rejection but may cause the relapse of the cancer.
Allogeneic BMT (Donor Transplant)
- Stem cells originate from a donor who is either a related or an unrelated person.
- It is used in the treatment of leukemia, aplastic anemia, other anemia forms, cataracts, genetic disorders, and some pernicious anemia.
- Higher risk of Graft-versus-Host Disease (GVHD).
Haploidentical BMT (Partially Matched Donor)
- Often, one of the parents or the sibling is only half related to the other parties involved due to the 50/50 matching.
- Increases the source of donors but has a more aggressive requirement of immune suppression.
Umbilical Cord Blood Transplant
- Umbilical cord blood stem cells are obtained from the umbilical cord which is cut soon after the birth of the baby.
- For pediatric patients and the patients that have no matched related donor.
Preparing for Bone Marrow Transplant
Medical Preparation
The suitability of the patient for the transplant is first judged according to his or her medical condition in the period prior to the transplant.
Pre-Transplant Tests & Evaluations
- Blood tests – Quick check of infection, internal body function and tissue matching.
- Bone marrow biopsy – Confirms disease status.
- Diagnostic tests like X-rays, CT scans, MRI to determine if there are any abscesses or other problems there.
- The heart and lung functions test would help determine whether the body can take the treatment in place.
- HLA (Human Leukocyte Antigen) Typing – Matches a donor for allogeneic transplants.
Conditioning Therapy (Pre-Treatment)
- It is also noteworthy that patients are subjected to high-dose chemotherapy and/or radiation therapy for specific purposes, which are to:
- Destroy diseased bone marrow.
- Immunosuppression when transplanting stem cells of the donor in an attempt to engraft properly.
- This we must do in order to make room for new stem cells for use, both in laboratory animal experiments and transplants in human beings.
- It is important to note that the intensity of this therapy varies depending on the disease as well as the type of transplant to be conducted.
Vaccinations & Infection Prevention
- Before the BMT, patient may be vaccinated against pneumonia, flu and hepatitis.
- These can either make their doctors prescribe antibiotics or anti-viral drugs to stop the formation of more boutiques.
During Bone Marrow Transplant
The procedure of the actual bone marrow transplant is another important process because in the last step, healthy stem cells are injected to the patient’s venous system for the replacement of unhealthy marrow. This stage does not require surgery and normally takes several hours.
Stem Cell Infusion (The Transplant Process)
- Stem cells are transplanted through a central vein catheter, a central IV catheter placed in the body’s largest veins in the chest.
- The process of infusing is quite like that of blood transfusion and requires 1-4 hours on average.
- It can be said that there is no need for anaesthetic since the process is painless.
Bone Marrow Transplant Risks
BMT is a life-saving treatment method but poses several risks and complications since the patient’s immune system is much weakened and afflicted by strong chemotherapy or radiation. It includes age, health condition, type of transplant, whether it’s auto or allo and compatibility of the donor’s marrow with the patient.
- Infections
- Graft Failure
- Mucositis

FAQ
1. What do BMT stand for and what is it all about?
- Bone marrow transplant is a medical procedure where patients are given healthy bone marrow stem cells that replace the ones that are damaged or destroyed. It is commonly done for the cases of leukemia, lymphoma, aplastic anemia, and other blood related disorders.
2. How long does it take to complete the whole process of transplant?
- It must be noted that the hospitalization period ranges from three weeks to six weeks on average for different patients. Recovery can take anything from several months to a year and above.
3. When it comes to Bone Marrow Transplants, what can be considered the success rate of this procedure?
- Overall success rates may depend on type of disease, availability of matching donor and general health. One kind of BMT is autologous BMT while the other is allogeneic BMT, and it was observed that BMT of the autologous type is more successful than the allogeneic type. The average five-year percent survival for some illnesses is between 50 and 80 percent.
