28-Apr-2025An allogeneic bone marrow transplant is a medical treatment wherein an individual is given healthy bone marrow donated by another person. The treatment is commonly employed to cure several blood cancers (e.g., leukemia, lymphoma, and multiple myeloma), some blood diseases (such as sickle cell anemia or aplastic anemia), and immune system disorders.
Testing and Evaluation: Before the transplant, the patient is subjected to a series of tests to determine their overall health and eligibility for the procedure
Chemotherapy and/or Radiation: Patients, in most instances, receive chemotherapy, radiation, or both to kill the cancerous cells or to condition the body to accept the new bone marrow. This is referred to as "conditioning."
Autologous BMT: Stem cells from the patient are harvested prior to conditioning. The stem cells are harvested by a technique known as apheresis where blood is tapped, and the stem cells are separated from other blood components, frozen, and stored.
Allogeneic BMT: Stem cells are taken from a matched donor. This may be a similar apheresis technique or, for some, bone marrow is actually removed directly from the donor's pelvic bones by a needle.
The stem cells that have been harvested are dripped into the bloodstream of the patient, just as a blood transfusion is given. The stem cells make their way to the bone marrow, where they start to proliferate and form healthy blood cells.
Engraftment is the process by which the transplanted stem cells start to develop in the bone marrow of the patient and begin to form new blood cells. This can take anywhere from a few weeks to a few months.
Throughout this period, the patient is susceptible to infections, bleeding, and anemia and will require close observation.
Discharge from Hospital: Most patients are discharged from the hospital following the initial post-transplant period, but they require close observation.
Engraftment Completion: At 3 months, the stem cells have usually fully engrafted, and the patient's blood counts (red blood cells, white blood cells, and platelets) are rebounding.
Decreased Risk of Infections: The immune system gradually recovers, although it will remain weaker than it was before transplanting. Patients are less likely to experience infections but must still exercise vigilance.
Regular Monitoring: Patients must attend regular follow-up for a number of years to check for signs of recurrence of disease (in the event of cancer treatment) or complications such as GVHD.
