Bladder cancer is normally grouped into stages depending on the extent of the spread of the cancer both in the bladder and into other parts of the body. TNM staging system is by far the most widely accepted system and remarkably, its stages are numbered 0 to IV (0 to 4). This is a breakdown:

Stage 0 (Papillary Carcinoma and Carcinoma in Situ)

  • Stage 0a (Non-invasive papillary carcinoma): The tumor mandates only on the lining of inner portion of the bladder.
  • The stage 0 is (Carcinoma in situ): This is a high grade tumour which is flat and is confined to the lining of the bladder. Considered aggressive.
  • No intrusion into lower levels. Highly treatable.

Stage I

  • It has spread to the connective tissue layer under the lining of bladder (lamina propria) but has not penetrated into the muscle layer.
  • Remains local, yet there is increased risk of its progression.

Stage II

  • The bladder wall muscle layer has been invaded with cancer.
  • Worse; greater chance of transmission. There may be a necessity of surgery or intensive therapy.

Stage III

Cancer has diffused through the muscle layer to the fatty medium surrounding it and possibly may include:

  • The male reproductive organs (prostate)
  • The Lady bit in women Womens womb or womb Vagina
  • Cancer that is locally advanced. The care usually involves surgery, chemotherapy, radiation.

Stage IV

Cancer has metasatasized to:

  • The locally advanced abdominal or pelvic wall (abdominal or pelvic wall)
  • Or even the separate organs such as the lung, liver or bones (metastatic)
  • Advanced cancer. It is sometimes not curable and treatment can prolong life with symptoms being alleviated.

Early Symptoms of Bladder Cancer

The early warning signs of bladder cancer may be helpful to the early detection of the disease. But such symptoms are also misdiagnosed as other non-threatening conditions such as urinary tract infections (UTIs).

Early signs and symptoms:

Blood in Urine (Hematuria)

  • Most common symptom
  • Urine can appear red, pink or cola-coloured
  • May be procedural (come and go)
  • In other cases only seen through a microscope (microscopic hematuria)

Frequent Urination

  • The increased urge to urinate with greater frequency than before

Urgency to Urinate

  • A great desire or feeling to urinate despite the bladder remain empty

Burning or Pain during the Urination (Dysuria)

  • Confused with the symptoms of UTI

Incomplete emptying feeling

  • This results in the feeling that even after passing water, not the entire bladder was emptied

Less frequent early signs:

  • Pelvic pain
  • On the one hand- lower back pain
  • Unexplained fatigue
  • Weight loss or lack of appetite

Chemotherapy

  • Muscle-invasive or metastatic cancer Systemic therapy
  • Adjuvant or neoadjuvant (used before or after surgery)
  • Ordinary chemicals: Cisplatin, Gemcitabine

Radiation Therapy

  • Chemoradiation is frequently used with chemotherapy.
  • Patients that are not able to receive surgery have an option

Immunotherapy / Targeted therapy

  • In cases of advance/metastatic disease
  • Medications: Pembrolizumab Atezolizumab (checkpoint inhibitors)
  • It is available in large Indian cancer institutions

Advanced Bladder Cancer Symptoms

When bladder cancer is diagnosed at the advanced stages (muscle-invasive or metastatic), it causes a more dramatic process in progressive symptoms based on the bodies and organs of interest.

Local Symptoms:

Pelvic Pain

  • Pain in the lower abdomen or pressure in the belly or down low

Painful Urination

  • Urinary burning or frequency of urination

Incontinence or Problem Urinating

  • Difficulty with starting or stopping a flow of urine
  • Automatic emptying (in case of damages of the bladder or the surrounding nerves)

Hematuria 

  • Hematuria which is persistent, and sometimes, darker or clotted

Obstruction by the bladder or Urethra

  • Potentially may lead to urine collection or swelling of the kidney (hydronephrosis)
