Myringoplasty

A myringoplasty is basically a surgery which is executed by an otolaryngologist generally to re-fix the hole in the eardrum. In this type if surgery, usually the hole is repaired by fixing a graft constituted of either a tiny or small piece of tissue from somewhere on the body, or a typical gel-like material. Your child’s eardrum which is also known as the typical tympanic membrane, is situated between his outer ear and middle portion of the ear. This basic thin membrane moves in answer to the sound waves and have a pivotal role in the hearing process.

A precise hole which is formed in this specific membrane from a critical ear infection or basically an ear tube or from an injury, like as an object majorly in the ear canal. A child can recognise this hole if he blow up his nose and hears the air coming out, if he feels the hearing loss, or if he sees fluid has particularly drained onto his pillow during the night.

In almost 90 percent of the cases, a precise hole is made in the eardrum heals without the properly executed treatment. Holes that generally do not close on their own in about three months are prone to infections and can clead to temporary hearing loss problem. In certain cases, the surgery is essential. In almost 95 percent of the myringoplasties performed, the surgery properly fixes the hole, minimises the risk of any further infections, and refine or greatly enhance hearing.

Other kinds of surgical approaches to eardrum repair involve : a kind of tympanoplasty, which involves a more intense reforming of the ear drum and separating the scar tissue, and generally an ossiculoplasty,Page Image which repairs or dusplaces the three small bones in the ear drum which are known as ossicles.

Need for Myringoplasty Treatment

Myringoplasty is a surgical operation on the tympanic membrane (ear drum) to close a perforation or hole. This treatment is normally advisable in patients having chronic ear perforations where there has been repeated tear of eardrum that fails to heal without help, hearing impairment, repeated attacks of ear infection and ear discharge.

Symptoms leading to Myringoplasty Treatment

Conditions that may lead to development of symptoms which may require myringoplasty normally originate from a torn eardrum or some complications of the ear.

Here are the key symptoms:

  • Hearing Loss
  • Recurrent Ear Infections
  • Persistent Ear Discharge
  • Ear Pain or Discomfort
  • Tinnitus
  • Vertigo or Dizziness
  • Difficulty Swimming or Bathing

What Complications can happen?

Myringoplasty treatment in India is successful in most of the cases but also has some complications and risks involved in it like-

  • Bleeding is common as a complication post surgery.
  • Blood clots are also frequently reported after the completion of the surgery.
  • Failure of the graph in certain cases
  • Wound on the surgical site is also reported.
  • Loss of hearing is rare but can be one outcome of the surgery.
  • Change of taste and allergic reactions are common post surgery.
  • Unsightly scarring is also one of the common risk of Myringoplasty.

Myringoplasty Ear Surgery Procedure

Myringoplasty treatment in India is carried in the following manner-

  • The otolaryngologist generally performs this procedure at one of the basic outpatient treatment center.
  • Your child will have to lie on the table generally with his face to the one side.
  • The surgeon will then implement a regular anaesthesia in this procedure that is basically between 10 and 30 minutes of time duration.
  • During the myringoplasty, the surgeon will make entry in the ear by an incision in the ear or specifically through the ear canal, relying on the size and place of the hole and the size of your child’s ear canal.

Use a basic instrument to refine the edge of the hole which stimulates body’s own natural treatment process-

  • First a graft made of tissue is placed from below his skin or gel foam which is paper-like material.
  • Then the child’s ear will be packed with cotton that will remain in his ear for about a period of a week.
  • After that, a bandage across the outside of the ear to safeguard his ear canal.

After the myringoplasty surgery:

  • Your child may feel sleepy or even nauseous and lethargic from the anaesthesia given before the surgery.
  • He can rehabilitate with his parents at his side in the post-operative cell.
  • He will most probably feel good within initial hours of the surgery and when he goes home, which is generally the day of surgery.
  • Then he will be prescribed some antibiotic ear drops to avoid the risk of infection; the drops which go into his ear canal, needing a momentary separation of the packing.
  • He can have bland foods and drink clear and soft liquids.
  • He can come back to daily routine activities at his own speed, generally within a few days.
  • And he will have some ear pain and soreness for the initial some days, but the uneasiness will shed within five days.
  • Acetaminophen will help, if the doctor suggests it.
  • He can also feel popping, clicking, shunting or other kinds of sounds in his ear, which are common and will shed away in a course of a few days.
  • Ear drainage can be maintained outside the ear bandage for the initial l days.
  • The doctor or the nurse will give you guidance on how this is to be performed.
  • There should not be anything in the ear canal except the accurate antibiotic ear drops.
  • Myringoplasty is to rebuild the tympanic membrane basically, in prevention to the middle ear and enhance the hearing.

What are the types of Myringoplasty?

Underlay technique

  • The temporalis fascia is generally a grafted type of surgical procedure.
  • A precise incision is made in the edge of the perforation and a ring of epithelium is separated.
  • Basically, a strip of mucosal layer is separated only from the inner side of the occurred perforation.
  • The middle ear is then packed with some gel foam soaked with an antibiotic.
  • The edges of the graft should come under the certain scales of the perforation and a tiny part will also be enlarged over the specific posterior canal wall.
  • The tympanomeatal flap is then re-located.

Overlay technique

  • In this kind of surgical process, the temporal fascia is harvested first.
  • A precise incision is done to intensify the medial meatal skin with the tympanic membrane of epithelium.
  • The graft is then placed on the outer surface of the tympanic membrane; generally a slit is done to tuck it under the handle of a specific malleus.
  • The ear is then packed with certain gel foam and antibiotics medications, and the precise incision is closed.
  • Eventually a mastoid dressing is executed.

Laser Myringoplasty

Myringoplasty through the use of a laser is referred to as Laser myringoplasty or Laser-Assisted Myringotomy (LAM) is applicable on cases of refractory otitis media with effusion or ears with perforation. The present technique of LAM is different from the normal myringoplasty, where the grafting process is done; LAM makes small holes or repairs minor meniscal in the eardrum using a laser.
