How does entropion or ectropion happen?

The problem is generally caused due to aging. Usually, ectropion or entropion is caused due to scar tissue pulling the patient’s eyelid out of their original position. Only the lower eyelid is generally affected.

Entropion is a condition characterized by the outward rolling of the eyelid margin, ordinarily the inferior one and the lashes are turned inwards to touch the eye.

Ectropion is a condition in which the eyelid (more often the lower one) is pulled outward, exposing the inner surface of the lid.

Surgical Repair of Entropion

Rotational Sutures:

  • Full-thickness eyelid sutures can be placed at the bedside or in the office as a quick procedure for offering the patient's immediate relief from the symptoms of entropion. 
  • The procedure usually takes an average of 15 minutes for the completion and local anaesthesia is only required. 
  • A double-armed 4-0 chromic suture is placed from the deep inferior fornix just underneath the inferior tarsal border, via the lower lid retractors, and the chromic suture exits via the skin superior to the level of insertion. 
  • Exiting more superiorly can result in more lid eversion. 
  • The rotational sutures procedure indirectly tightens the lower lid retractors and forms scarring between the retractors, orbicular and the skin for preventing the override.

Tarsal Strip

  • Horizontal lid laxity can be addressed surgically with the help of a tarsal strip. 
  • A lateral canthotomy and inferior cantholysis are done for releasing the lower lid. 
  • The mucocutaneous junction is being removed from the lid margin, and the anterior lamella is being excised off the tarsus. 
  • Conjunctiva is being debrided from the posterior tarsus. 
  • A horizontal cut is made through the superior tarsal plate creating a strip that is trimmed for achieving the desired amount of tightening. 
  • The strip is then attached to the internal aspect of the lateral orbital rim.

Lower Lid Retractor Reinsertion

  • Lower lid retractor disinsertion can be reversed by the help of the operation with posterior advancement of the retractors. 
  • After inferior cantholysis and lateral canthotomy, a cut is being made through the lower lid retractors and the conjunctiva spanning from the lateral canthus to just lateral to the inferior punctum, taking advantage of the bloodless plane just underneath the inferior tarsal border. 
  • The retractors and the conjunctiva are then separated from the anterior lamella for exposing the orbital septum overlying orbital fat. 
  • A strip of orbicularis is being excised or ablated inferior to the tarsus, this reduces the effect of preseptal orbicularis override. 
  • The patient can look up and down to assist in locating the retractors. 
  • The retractors are separated from the conjunctiva with the help of cautery, and sharp dissection allows for exposure of the anterior surface of the inferior tarsal plate.

Ectropion Diagnosis and Treatment

  • Lower eyelid laxity: The lower eyelid is horizontally tightening by use of a lateral tarsal strip or by a similar procedure.
  • Lower eyelid retractor disinsertion: The Jones procedure of reattaching the retractors to the tarsus.
  • Punctal ectropion: The medial spindle procedure reopposing the everted punctum.
  • Cicatricial ectropion usually requires the lengthening of the anterior lamella by skin grafting.
  • Paralytic ectropion usually requires horizontal tightening and correcting the punctal ectropion. With facial nerve paralysis, brow ptosis, and corneal exposure may also be required to be addressed.
  • Mechanical ectropion from facial ptosis may usually be required for surgical elevation of the patient’s midface, or a facelift in conjunction with lower lid tightening.
  • In some of the cases of heavy facial tissues or recurrence, periosteal fixation may not be given the desired result and therefore additional fixation may be required by the use of bone tunnels or bone plates to which the lower lid can be suspended.

What are the benefits of entropion and ectropion repair?

The patient’s eyelid should be in a better position and their symptoms should improve after the surgery.

How soon will I recover?

  • The patients should be able to go to their home after a few hours after the surgery.
  • The patients should not get their eyelid wet; the patients must perform a strenuous exercise or bend down until the stitches are being removed.
  • The patients should not drink alcohol or wear eye make-up for a few weeks, and the patients must keep their face out of the sun.
  • Regular exercise should help the patients return to their day to day activities as soon as possible. Before the patients start to exercise, they must ask their GP or their healthcare team for advice.
  • The results of an ectropion or entropion repair last for a long period of time. However, as the people get older the soft tissues of their eyelids and skin will continue to slacken and the problem may come back after a period of time.

Are there any alternatives to the entropion and ectropion repair surgery?

Artificial tears and ointments can be used for protecting the surface of the patient’s eye. The surgeon may suggest a special soft contact lens for the patients to wear. If the patients have entropion, the surgeon may suggest the use of tape for preventing the patient’s eyelashes from rubbing against the patient’s eye, injecting Botox into the muscle that turns the patient’s eyelid inwards, or use of the stitches for preventing the patient’s eyelid from turning in.

Success Rate of Entropion and Ectropion Repair Treatment

Both entropion repair and ectropion repair differ in terms of success, with surgical methods having 90%-95% success rate and 85%-95% success rate respectively. As such, success factors include the source, the degree of the problem, the quality of surgical work done, and the post-operative care.
