Coronary Angioplasty Treatment

Stent Procedures

A coronary angioplasty is a procedure used to widen blocked or narrowed coronary arteries (the main blood vessels supplying the heart). The term 'angioplasty' means using a balloon to stretch open a narrowed or blocked artery.

What is Coronary Angioplasty?

A coronary angioplasty is a procedure used to widen the blocked or narrowed coronary arteries. The term 'angioplasty' means using a balloon to stretch wide open a narrowed or blocked
artery. However, most modern angioplasty procedure involves inserting a short wire-mesh tube, known as stent, into the artery during the procedure. The stent is left in place
permanently to allow the blood to flow more independently. Coronary angioplasty is sometimes also known as percutaneous transluminal coronary angioplasty. The combination of
coronary angioplasty with stenting is usually known as percutaneous coronary intervention. Angioplasty is done on more than a million people per
year in the United States. Serious complications don't occur usually, but can happen no matter how careful your doctor is, or how well he or she does the procedure.

What are the benefits of coronary angioplasty?

In most of the cases, the blood flows through the coronary arteries improves after an angioplasty. Many people find that symptoms get significantly better and they are able to do
more than they could before the procedure. If you've had a heart attack, an angioplasty can increase the chances of surviving more than clot-busting medication. This procedure
can also reduce the chances of having another heart attack in future.

A coronary angioplasty is one of the most common type of treatment for the heart.
Coronary angioplasties are most commonly performed in people aged sixty or older, as they are more likely to have heart disease.
As the procedure does not involve making major incisions in the body, it is usually carried out safely in most people. Doctors refer to this as a minimal invasive form of treatment.

The risk of serious complications from coronary angioplasty is in general small, but this depends on factors such as:

  • Person's age.
  • Person's general health.
  • Whether he had a heart attack.

Serious problems that can occur as a result of this procedure include

  • Excessive bleeding.
  • Heart attack.
  • Stroke.

Why do I need a coronary angioplasty?

Like all organs in body, the heart needs a constant supply of blood. This is supplied by the coronary arteries. In older people, these arteries can become narrowed and hardened,
which may cause coronary heart disease. If the flow of blood to the heart becomes restricted, it can lead to chest pain called angina, which is usually
triggered by a physical activity and/or stress. While angina can be treated with medication, a coronary angioplasty may be required to restore the blood supply to heart in
severe cases where medication is generally ineffective. Coronary angioplasties are also used as an emergency treatment after a heart attack.

What does the operation involve?

A coronary angioplasty is performed using a local anaesthetic, which means you will be awake while the procedure is carried out. A thin flexible tube, a catheter will be inserted
into one of your arteries by an incision in your groin, wrist or arm. This is guided to the affected coronary artery using X-ray video. When the catheter is in it's place, a thin
wire is guided down the length of affected coronary artery, delivering a small balloon to the affected section of artery. This is then inflated to widen up the artery, squashing
fatty deposits against the artery wall so that blood can flow through it more freely when the deflated balloon is removed. If a stent is being used, this will be near the balloon
before it is inserted. The stent will expand when the balloon gets inflated and remains in place when balloon is deflated and removed. A coronary angioplasty usually takes about
30 minutes to two hours. If you are being treated for angina, you'll normally be able to go home later, the same day or the day after you have the procedure. You'll need to avoid
heavy lifting, strenuous activities and driving for at least a week or two.

What Are the Risks of Coronary Angioplasty?

Coronary angioplasty is a common medical treatment. Although angioplasty is normally safe, there is a small risk of complications, such as:

  • Bleeding from blood vessel where the catheter was placed.
  • Damage to blood vessel from catheter.
  • An allergic reaction to dye which is given during the angioplasty.
  •  An irregular heartbeat.
  • The need for emergency coronary artery bypass grafting during this procedure. This may occur when the artery closes down instead of opening up.
  •  Damage to kidneys caused by the dye used in the procedure.
  • Heart attack.
  • Stroke.

As with any procedure involving the heart, complications can sometimes, though very rare, can cause death.
Sometimes chest pain can occur during angioplasty because the balloon briefly blocks off the blood supply to the heart.

The risk of complications is higher in:

  • People aged seventy years and older.
  • People who have kidney disease and/or diabetes.
  • Women.
  • People who have poor pumping function of heart.
  • People who have serious heart disease and blockages.
  • Research on angioplasty is ongoing to make it safer and more effective, to prevent the treated arteries from closing again.

Tips for Healthy Diet After Angioplasty:

Foods to Include in the Diet-

  • Whole grains.
  • Fruits and vegetables.
  • Healthy fats.
  • Nuts, beans, and seeds.
  • Dark chocolate.
  • Fresh-brewed tea.

Are there any alternatives?

If many coronary arteries are blocked and narrowed, or the structure of your arteries is abnormal, a coronary artery bypass graft may be considered.
This is a type of invasive surgery where the sections of healthy blood vessel are taken from other parts of body and attached to coronary arteries. Blood is diverted through
these vessels, so it by-passes the narrowed or clogged parts of the arteries.

Coronary Angioplasty Treatment in India

An angioplasty is a surgical procedure that is primarily used to open the blocked cardiac arteries. This is typically done as an emergency procedure, frequently right after a person has a heart attack. Coronary angioplasty is also known as balloon coronary angioplasty and percutaneous transluminal coronary angioplasty. To increase artery width and enhance blood flow, a little balloon is inflated inside the vessel. To maintain the artery open, a mesh tube known as a stent may occasionally be implanted.

Need of an angioplasty procedure

Deposits called plaque can constrict or obstruct arteries. The accumulation of fat and cholesterol on the interior of arterial walls is called plaque. This medical condition is known as atherosclerosis, or the hardening of the arteries.

Angioplasty is a treatment option for:

  • Coronary artery blockage that occurs during or following a heart attack.
  • Blockage or constriction of one or more coronary arteries, which can result in heart failure or other heart problems.

  • Narrowings that impair blood flow and result in angina, a chronic chest pain that is uncontrollable despite medication.

Angioplasty process in India

The following steps are involved during an angioplasty:

  • Administering local anesthesia to patients.
  • A catheter with a tiny inflatable balloon at the end is first introduced into the damaged artery through an incision made in the arm or groin.

  •  Using specialized dyes and X-ray imaging, medical professionals guide the catheter into the clogged coronary artery.

  • The balloon is inflated once it is in place to enlarge and widen the artery.

  • By pushing the plaque (fatty deposits) up against the artery wall, blood flow is made possible.

  • In some instances, the catheter also includes a stent, which is a mesh made of stainless steel.

  • Even after the balloon is deflated and removed, the stent can stay in place and continue to hold the blood vessel open.

Types of Percutaneous transluminal angioplasty done in India:

Cardiac surgeons in India perform various following angioplasty techniques

Balloon Angioplasty

To broaden and restore blood flow, a small balloon is inflated inside the restricted artery. Drug-eluting balloons (DEB) are utilized in advanced procedures to administer treatment directly to the site of injury. During angioplasty, the DEB is inflated at the blockage site to release the drug into the artery wall. This medication prevents the overgrowth of cells that lead to restenosis.

Stent placement

A tiny mesh tube known as a stent is frequently implanted following balloon angioplasty to strengthen the artery and prevent it from narrowing.

The heart's tissues will begin to cover the stent like a layer of skin, within some months, the skin will completely envelop the stent. Stents can be categorized as follows based on their characteristics:

  • Bare metal stents (BMS): A stent is a mesh-like tube created from thin, uncoated (bare) metal wire.
  • Drug-eluting stents (DES): These stents, like conventional stents, are coated with medications to stop potential artery constriction.

  • The bioresorbable scaffold system (BRS): This stent dissolves entirely and leaves no trace of the stent material in the artery after the obstruction within the channel is effectively removed and healed.

Cutting Balloon Angioplasty

It expands certain resistant plaques that are difficult to push back by making longitudinal incisions using a balloon equipped with microscopic blades.

Atherectomy

In this, a little incision is made in order to introduce a catheter. A revolving gadget, laser, or sharp blade is attached to its other end. Plaque is then scraped away, dissolved, or broken up without creating a major incision with the aid of its opposite end.

Post surgical care after Angioplasty in India

The following precautions are recommended by cardiac surgeons to guarantee the success of angioplasty.

Drugs: Certain drugs are used by cardiac surgeons to lower inflammation, avoid blood clots in the stent, and enhance long-term results for patients with coronary artery disease. This blood clot and ongoing inflammation could cause a heart attack or other deadly outcomes if they are ignored.

Heart rehabilitation: Numerous hospitals provide fitness instruction and general health monitoring as part of their rehabilitation programs. Under the supervision of the physician, this guarantees a quicker recovery for the patients.

Lifestyle modifications: Doctors urge patients to adopt a healthy lifestyle, such as the following, to lower their risk of developing more cardiac problems.

  • Eating a balanced diet
  • Keeping a constant body weight

  • Get a sufficient quantity of rest

  • Avoid consuming alcohol

  • Quit smoking
