Pre-Transplant Evaluation:
Medical Assessment:
The patient is evaluated thoroughly before the transplant, with blood work, imaging studies, and physical exams. This assists in determining the patient's general health and tolerance for the transplant process.

Stem Cell Collection (Harvesting):
Bone Marrow Harvesting:
When stem cells are being collected from bone marrow, a procedure is done under local anaesthesia (or general anaesthesia) to harvest stem cells from the patient's hip bone. A needle is placed into the bone to harvest marrow, which has the stem cells.
Peripheral Blood Stem Cell Harvesting:
Stem cells are more often harvested from the patient's peripheral blood. Before collection, the patient receives growth factor injections (most commonly G-CSF) to induce the bone marrow to produce stem cells and release them into the blood. Blood is extracted from the patient and run through an apheresis machine that isolates the stem cells from the rest of the blood. The stem cells are harvested, and the other blood is returned to the patient.
Stem Cell Storage:
Once collected, the stem cells are processed carefully and kept frozen until use. This preserves the cells in a living state, waiting to be reinfused.
Conditioning Therapy:
High-Dose Chemotherapy and/or Radiation:
As a prelude to the stem cells being reinfused, the patient is given conditioning therapy to kill off any remaining cancer cells and put the immune system into remission. This usually consists of high-dose chemotherapy and, occasionally, radiation treatment. The objective is to eradicate the patient's cancer or underlying disease but to also kill off the patient's bone marrow, which will be replaced by the stem cells.
Stem Cell Infusion
Infusion Process:
After the conditioning therapy is finished, the stem cells that have been stored are thawed and infused back into the patient's blood. This is usually done via an intravenous (IV) infusion, much like a blood transfusion. The procedure usually takes a few hours.
Engraftment:
Within the next few weeks, the infused stem cells migrate to the bone marrow and start to form new blood cells (red blood cells, white blood cells, and platelets). This is referred to as engraftment. It may take weeks for the stem cells to fully restore the bone marrow, which is why patients are kept under close observation for infection, bleeding, or other complications.
