Preparation

Patient Evaluation

  • The physician looks at the foreskin to ensure the presence of phimosis or paraphimosis.

The Surgical Steps

Step 1 – Marking the Incision

  • A vertical line is drawn on the back (top) of the foreskin.
  • The length is determined by the tightness of the foreskin - normally 1-2 cm.

Step 2 – Clamping

  • On both sides of the intended line, two little clamps are used.
  • This helps reduce bleeding.

Step 3 – Making the Dorsal Slit

  • The surgeon cuts a vertical line on the outlined line.
  • The constriction band of foreskin is emancipated.
  • In the case of paraphimosis, this follows and liberates the trapped glans.

Step 4: Hemostasis (checked bleeding)

  • Micro bleeds are treated with cautery or pressure.

Step 5 – Suturing (if required)

The margins of the incision can be:

  • Stitch sewed to form a fluent aperture, OR.
  • Unanswered in case it is an emergency procedure.
  • Ordinarily absorbable stitches are used.

Step 6 – Dressing

  • Light sterile dressing is used.
  • To reduce the risk of infection, a layer of antibiotic ointment may be placed. 

Duration

  • The process often requires 10-20 minutes.
  • Carried out as a day-care process (no hospitalisation required).

After the Procedure

Immediate Care

  • Patient is allowed to come home the same day.
  • The dressing should be left the first 24-48 hours.

Pain & Swelling

  • Light pain, swelling, or bruising is also normal within several days.
  • They are prescribed pain medications and ointments.

Hygiene

  • Keep the area clean and dry.
  • Warm saline bath can be recommended subsequent to 48 hours.

Activity Restrictions

  • Sex or masturbation should be avoided in 4-6 weeks.
  • Wear loose clothes to minimize friction.

  • Tight clothing or incision irritating jeans should be avoided.

  • Swelling reduces.
  • Sutures (in case present) begin to loosen.

Week 2

  • Wound begins to close.

Activity Restrictions

  • Sexual intercourse and masturbation should be avoided within 4-6 weeks as a way of avoiding reopenings of wounds.
  • However, do not have strenuous activities and heavy lifting within a period of 1 week.
  • To prevent infection, do not swim or use hot tubs in 2 weeks.

Pain Management

  • Mild pain is common.

Use:

  • Paracetamol or other prescribed painkiller. 
  • Cold compress (intermittent but not over the skin)
  • Use NSAIDs only in cases when the doctor cautions you (they can make you bleed more).
