Ilizarov Limb Lengthening

Ilizarov limb lengthening is a surgical method that they apply to the bones – usually the legs – in order to increase the length of the limb or to correct any malformation. It employs some technique such as the Ilizarov method or the more advanced internal fixators (for example the Precice nail) to gradually extend the bones of the legs.

Ilizarov Technique

The Ilizarov technique is a surgical technique which was invented by a Russian orthopaedist Gavriil Ilizarov. It is mainly indicated in limb lengthening, correction of bone deformities, fractures, even in cases of infection or tumour resection and bone transport. The technique operates with the notion of distraction osteogenesis which is the natural process of bone regeneration in the body if two sides of a bone are pulled apart gradually.

The Ilizarov technique is made up of several key parts:

  • Osteotomy: An incision is then made on the bone, as carefully as possible and with the least possible damage.
  • Ilizarov External Fixator: A circular metal frame with the wires and pins passing through the skin and the bone is connected to the limb.
  • Gradual Distraction: There is a slight waiting time that ranges from 5-7 days and then the device is slightly tightened daily, about 1mm a day to widen the gap between the bones.
  • Bone Regeneration: As the gap are slowly stretched, new bone forms in the gap.
  • Consolidation: After obtaining the desired length or the degree of correction, the frame stays in the place and the new bone toughens and strengthens.

Limb Lengthening Surgery

Limb lengthening also known as bone lengthening surgery is a surgical technique aimed at increasing the length of the limbs, such as legs or arms. It is both a medical (for example it is used for the lengthening of limbs due to various causes such as diseases, deformities, or injuries) and a cosmetic procedure (for those who wish to be taller for no medical reason). It generally operates on the notion of distraction osteogenesis, where the bone is pulled apart to promote the growth of new tissues.

Core Steps of Limb Lengthening Surgery

  • Osteotomy: The surgeon hacks through the bone (femur or tibia).
  • Device Insertion:
  1. External Fixators (e.g., Ilizarov frame)
  2. Internal devices: Internal fixation devices encompass devices such as Precice nail, FITBONE which are used internally to fix the bone fractures.
  • Distraction Phase: Five to seven days later, the bone is gradually separated at a rate of 1mm per day.
  • Consolidation Phase: The new bone forms in several weeks or months of bone remodeling process.
  • Rehabilitation: It may consist of physiotherapy and pain control, and monitoring the condition via X-ray tests.

Leg Lengthening Procedure

Preoperative Evaluation

  • X-rays, MRI, blood tests
  • Evaluation of the bone density and joint conditions and muscular mobility
  • Goal setting: I want some information regarding how many cm of height increase.

Surgical Bone Cut (Osteotomy)

  • The surgeon has to make an incision on the femur popularly known as the thigh bone or the tibia which is the shin bone.

Implantation of Lengthening Device

  • Spinal Fixation (e.g., Harrington Rod, Kohler Buckle, Texas Scottish Rite, Milwaukee Brace)
  • Space for nails Interosseous (e.g., Precice, FITBONE, LON).

Latency Period

  • At least five to seven days before the initiation of the distraction process.

Distraction Phase

  • Gradual lengthening: 0.75–1 mm per day
  • Can be done manually, such as with an external fixator or through magnetic means utilizing the Precice device.
  • They take approximately 6-12 weeks on average depending on the height one wanted to achieve.

Consolidation Phase

  • New bone (regenerate) hardens
  • Takes 1–3 months per cm lengthened    

Device Removal (if applicable)

  • S Screws may still be left or retrieved 1–2 years after implantation
  • In this sense, the external fixator is often applied when the bone might need more support, and is usually removed after the consolidation of the new bone.

Rehabilitation

  • Muscle and joint intensive physical work
  • Regular imaging to monitor healing

Ilizarov Apparatus

The Ilizarov apparatus is an external fixator circular device that helps in limb lengthening, correction of deformities, as well as management of compound fractures and defective limbs.   • Rehabilitation : Ongoing (6–12 months) - Physical therapy to restore motion and strength
